Primary Care Networks (PCN) Knares Medical Practice is working together with practices in West Basildon PCN to improve patients quality of care.

Primary care networks were introduced in January 2019, which gave the opportunity for GP practices to join networks each with between 30,000 and 50,000 patients.

The Knares Medical Practice is in West Basildon PCN (WBPCN) made up of five practices in Basildon with a population of approximately 50,000 patients.

ln order to support Practices, Additional Roles Reimbursement Scheme (ARRS) was established, to provide a multi-disciplinary teams to provide more integrated health and social care services locally. The patients are able to benefit from the following roles of healthcare providers-

Clinical Pharmacist, Pharmacy Technician, Social Prescribing Link Worker, Health and Wellbeing Coach, Care Coordinator, Physician Associate, First Contact Physiotherapist, Dietician, Podiatrist, Occupational Therapist, Trainee Nursing Associate, Paramedics, Advanced Practitioner and Advanced Mental Health Practitioner.

With these additional workforce, The Knares Medical Practice will be able to book patients onto those additional roles, to enable the GPs and and Nurses to devote more time to other quality of services framework.

How to get a prescription

pills1If you think you may need a prescription medicine, please make an appointment with the doctor. Our practice nurse/nurse practitioner can also issue prescriptions for common ailments. If you have previously been given a prescription for a straightforward condition, such as hay fever, we may be able to issue a prescription after a telephone consultation with a doctor. Please ask the receptionist.

Your local pharmacist should be able to give you advice for non-prescription medication. Ask us about registering for our dispensing service if you think you qualify.

How to get a repeat prescription

YOU CANNOT REQUEST A REPEAT PRESCRIPTION BY EMAIL

We have a computerised repeat prescription service. Repeat prescriptions are normally for patients with long-term conditions who receive regular treatment. Your doctor (or nurse practitioner) will decide who can have a repeat presecription. From time to time we will ask you to see your doctor (nurse practitioner) to review your medication. We do this to monitor your illness and medication.

You can ask for a repeat prescription: by attending the surgery and completing a form when the reception is open or by post or fax. Wherever possible please give the exact drug names when ordering. Please allow 72 hours for your prescription or if it is something not normally on your repeat list it may take longer.

We cannot take telephone requests for repeat prescriptions because of the possibility of error.
